For the convenience of our new patients, we’ve provided you with documentation you can print out and complete to bring with you to your first appointment. This will make the process go more efficiently for you and save you time at the office.
All copays are due at the time of your appointment. Please be sure to check with your insurance company regarding deductibles or co-insurance payment prior to your appointment. We accept MC, Visa, Discover, Cash or Check.
